Job summary

Preferred Home Health Care & Nursing Services in Illinois seeks a licensed Physical Therapist to evaluate and treat patients of all ages in home health settings, developing and adjusting individualized care plans based on patient response and evidence.

You will act as a clinical resource for staff and students, coordinate with referring providers, and ensure safe patient transfers, proper body mechanics, and effective therapy modalities.

Qualifications

• Bachelor’s Degree in Physical Therapy and licensure as a Physical Therapist (PT) in Illinois.
• BLS certification within 30 days.
• Strong knowledge of body mechanics, patient assessment, and therapeutic modalities.
• Empathy, professionalism, and excellent communication skills.
• Ability to work independently and as part of a multidisciplinary team.

Responsibilities

• Designs and adjusts treatment plans to meet patient goals, outcomes, and discharge needs.
• Educates patients, families, caregivers, and healthcare team members using effective teaching methods.
• Performs clinical evaluations, selecting appropriate examination techniques relevant to diagnoses.
• Documents care in compliance with policies and monitors the effectiveness of therapy services.
• Utilizes support personnel ethically and provides feedback on their performance.
• Completes necessary continuing education, particularly for acute stroke care, and manages therapeutic modalities such as ultrasound and iontophoresis.
• Evaluates impairments related to orthopedic and neurologic conditions, conducting objective testing and functional assessments.
• Communicates progress and treatment recommendations with referring providers.
