Centra Healthcare Solutions is seeking an experienced Physical Therapist (PT) licensed or in the process to obtain licensure in IL to work in SNF/LTC settings on a contract basis. A minimum of 1 year SNF/LTC PT experience and a current IL PT license are required.

Centra Healthcare Solutions is seeking an experienced Physical Therapist (PT) that is licensed or in the immediate process, and qualifies, of obtaining Physical Therapist (PT) licensure in the state of IL to work in the specialty area of SNF / LTC.

This is a contract position5x8 Days schedule.This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Minimum Requirements include:

• 1 year of recent experience as a(n) Physical Therapist (PT) SNF / LTC experience required.Current Physical Therapist (PT) license within the state of practice.
