Job summary

Centra Healthcare Solutions Inc is seeking an experienced or newly graduated Physical Therapist for a contract assignment in a SNF/LTC setting. The 8am–4pm schedule supports predictable hours and strong team collaboration.

The PT will develop a plan of care, provide skilled therapy per physician orders, assess and modify care, and work with all disciplines to meet patient goals. Centra offers competitive pay, housing options, and CEU reimbursement.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist or in the licensure process.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services per physician orders.
• Assess patient needs, plan, evaluate and modify care to meet goals.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.

A great SNF / LTC is looking for an experienced or newly graduated Physical Therapist / PT to join their growing team for a contract assignment!This is a contract position.8am - 4pm schedule.This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Requirements include:

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or proactively in the process of the application process for current state licensure as a Registered Physical Therapist / PT.
• Develop a plan of care for each physical therapy patient.
