Responsibilities

The Physical Therapist will primarily provide outpatient rehabilitation services in an in‑home setting for the older adult population. The in‑home setting could be a patient’s private residence or in a patient’s room in an independent living facility. Powerback Rehabilitation To You allows the Physical Therapist to evaluate and treat in the most functional environment which is the patient’s home. The Physical Therapist assesses the need for, develops, and delivers physical therapy programs to facilitate rehabilitation. The Physical Therapist identifies the need for and makes referral(s) to other disciplines to address the comprehensive needs of patients.

Primary responsibilities include:

  • Providing individualized therapy programs to restore, reinforce, and enhance performance.
  • Implementing and adjusting patient care plans based on assessments and therapeutic goals.
  • Instructing, educating, and training patients and caregivers in skills needed to promote independence.
  • Directing activities that promote and maintain health.
  • Supervising Physical Therapy Assistants, temporary licensees, students, and support personnel as necessary.
  • Maintaining adherence to department and patient treatment schedule, clinical and billing documentation requirements.
  • Participating in patient care, staffing conferences, and related meetings.
  • Referring to other disciplines and agencies to address comprehensive patient needs.
  • Consulting with rehabilitation team members to develop appropriate therapeutic regimens.
  • Adapting program and/or plan of care to individual patient needs.
  • Promoting a safe environment and addressing fire and safety hazards.
  • Ordering, maintaining, and reporting maintenance of equipment.
  • Providing the highest quality of service to patients and families.
  • Screening, examining, and evaluating patients; history, systems review, application of appropriate tests and measures; diagnosis, impairments, precautions/contraindications and planning of care.
  • Designing and implementing treatment goals and plans in collaboration with patients and caregivers.
  • Documenting services and maintaining records.
  • Applying therapeutic techniques including therapeutic exercise (aerobic conditioning, balance coordination, agility training, body mechanics and postural stabilization, flexibility exercises, gait and locomotion training, neuromotor development training, relaxation, strength, power and endurance training), manual therapy, airway clearance techniques, integumentary repair and protection, use of electrotherapeutic, physical agent and mechanical modalities, health and wellness education, chronic disease management, falls assessment and interventions to reduce fall risk, and case management in the home and community.
  • Designing, fabricating, fitting, and applying adaptive devices, and training patients in their use.
  • Selecting activities suitable for patients’ physical, mental capacity, level of intelligence and interests.
  • Performing routine reexaminations to modify or progress the plan of treatment.
  • Incorporating health and wellness programming into treatment plans.
  • Performing home and community mobility assessments and providing recommendations for safe mobility in the home and community space.
  • Performing other related duties as required.
Qualifications
  • Valid Physical Therapy license in the state(s) of practice or proof of license eligibility.
  • Master’s degree in Physical Therapy, or a Master’s degree in a Physical Therapy related field in addition to a Bachelor’s degree in Physical Therapy; or a Bachelor’s degree in Physical Therapy plus 5 years of progressively responsible PT experience.
  • Graduate of a degree earned outside the U.S. must be evaluated as equivalent to a U.S. Physical Therapy degree.
  • Valid driver’s license or reliable, independent transportation to the patient’s residence.
  • Posted Salary Range: USD $79,000.00 – $110,000.00 per year.
