Job summary

Trilogy Health Services is seeking a Physical Therapist to provide comprehensive PT services in a senior living setting. You will assess, plan, and implement therapies in an interdisciplinary environment, while maintaining documentation and ensuring compliance with standards.

Education requires a Master Degree in Physical Therapy and current licensure. Travel is occasionally required as part of patient care across sites, with benefits starting after the first month.

Qualifications

• Master Degree in Physical Therapy.
• Current, valid state PT licensure or certification.
• CPR certification preferred.
• Travel required, typically less than 10%.

Responsibilities

• Conduct thorough assessments to evaluate patients' physical conditions and rehabilitation needs.
• Develop and implement personalized treatment plans with exercises and modalities.
• Educate patients and families on exercises, posture, and body mechanics.
• Maintain accurate documentation of progress and outcomes per policies.
• Collaborate with an interdisciplinary team to ensure comprehensive care.
• Other duties as assigned.
