Job summary

Qualifications

• Licensed Physical Therapist in the State of Illinois (or eligible for licensure).
• Minimum one year of clinical physical therapy experience or strong clinical affiliation experience.
• Valid driver's license and reliable transportation.
• Basic computer proficiency, including email and Microsoft Office applications.
• Strong communication, documentation, organizational, and time-management skills.
• Ability to work independently and manage a geographically assigned caseload.

Location: Southwest Suburbs Service Area Chicago Ridge, Oak Lawn, Bridgeview, Hometown, Hickory Hills, Justice, Burbank, Willow Springs, Bedford Park, Summit, Argo, Evergreen Park, and Chicago zip codes 60652 and 60655.

Schedule: Full-Time | Days, 8am-5pm Mon thru Friday with a weekend rotation

Position Overview

Advocate Health is seeking a compassionate and motivated Physical Therapist to join our Home Health team. As a Home Health PT, you will work independently while being supported by a highly respected and integrated healthcare organization. You'll provide skilled therapy services in patients' homes, helping them improve mobility, safety, independence, and overall quality of life. Physical Requirements

• Extensive travel throughout the assigned service area in all weather conditions.
• Ability to lift and carry up to 35 pounds without assistance and utilize patient handling equipment when appropriate.
• Ability to bend, stand, sit, reach, push, pull, climb stairs, and assist patients with transfers and ambulation.
• Ability to safely perform patient treatments within home environments.

Compensation

• Annual Base Salary: $90,064 - $135,096. This position is also incentive eligible. Actual compensation will be based on experience, qualifications, skills, training, and internal equity.
• Sign-on bonus: $10,000* (if applicable)
• Shift, on call, and more based on a teammate’s job.
