Job summary

Choice Home Health Monterey is seeking a dedicated Physical Therapist to deliver patient-centered home health services across Salinas and South Monterey County. You will perform initial evaluations, create and update plans of care, and conduct skilled PT interventions in patients’ homes.

The role emphasizes timely documentation, OASIS submissions when applicable, and collaboration with physicians, providers, and the care team to ensure high-quality outcomes.

Qualifications

• Master's degree in Physical Therapy.
• Active Physical Therapy license in good standing in the state of California.
• At least one year of experience working as a Physical Therapist.
• Must have a vehicle, a valid driver’s license, and automobile insurance.
• Ability to make home visits and provide physical therapy evaluations and treatments in the home environment, with timely documentation.
• Home Health and Home Care Home Base documentation experience.

Responsibilities

• Conduct comprehensive initial evaluations and assessments and promptly report findings to the physician/provider and Clinical Manager.
• Develop, implement, and update individualized plans of care based on patient needs and physician orders.
• Perform skilled physical therapy interventions, procedures, measurements, evaluations, and diagnostic tests in the home setting.
• Complete initial evaluations, treatment documentation, and re-evaluations every thirty (30) days; update plans of care as needed.
• Complete comprehensive assessments and OASIS documentation accurately and on a timely basis, as applicable.
• Observe, document, and report patient response, progress, and any significant changes to the physician/provider and care team.
• Communicate and collaborate with physicians, providers, and interdisciplinary team members to ensure coordinated, high-quality care.
• Provide education and support to patients and their families to promote safety, independence, and long-term success.
• Assist with orientation, supervision, and consultation for Physical Therapy Assistants, as assigned.
