Job summary

Enhabit Home Health & Hospice is seeking a licensed Physical Therapist to evaluate and treat patients in home health settings. You will work to improve mobility, independence in activities of daily living, and patient safety using evidence-based modalities.

The role emphasizes care coordination with families and care teams, participation in Specialty Programs such as Balance and Fall Prevention, and documentation in patient records. A valid state PT license and driving is required.

Qualifications

• Must be currently licensed as a Physical Therapist in the state of employment.
• Minimum of one year of clinical experience preferred.
• Demonstrate knowledge and skill in current therapy practice.
• Possess a valid state driver's license and automobile liability insurance.

Responsibilities

• Evaluate and treat patients with physical impairments using modalities.
• Aim to improve mobility, independence in daily living and safety.

Responsibilities

Our Physical Therapists (PTs) examine and treat patients with physical impairments through the use of physical modalities. The goal is to assist persons who are physically challenged to improve mobility and function, independent self‑care, and other skills necessary for functioning in daily living. PTs use our Specialty Programs, including Balance and Fall Prevention, Spine Safety, Total Hip and Knee Replacement Program, and Total Shoulder Replacement Program to treat patients.

Qualifications
