Job summary

VitalCaring is seeking a Physical Therapist (PT) for Home Health in Port St. Lucie, FL. This PRN role offers autonomous patient care delivered in patients’ homes and a chance to make meaningful differences in independence and quality of life.

You will evaluate, plan, and perform patient-centered therapy, educate families, and collaborate with the care team to optimize outcomes in a home-based setting.

Qualifications

• Graduate from an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice (Florida).
• Minimum 1–2 years of home health experience preferred.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans with physicians and the care team.
• Provide hands‑on therapy to improve mobility, strength, balance, and independence.

What You’ll Do

Deliver Patient-Centered Care

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential
• Develop and implement individualized care plans in collaboration with physicians and the care team
• Provide hands‑on therapy to improve mobility, strength, balance, and independence
• Educate patients and caregivers on treatment plans, exercises, and safety strategies

Drive Outcomes & Recovery

• Monitor patient progress and adjust treatment plans as needed
• Support safe transitions of care and contribute to discharge planning
• Identify additional patient needs and collaborate with interdisciplinary teams
• Partner closely with nurses, therapists, and physicians to ensure coordinated care
• Document patient care and progress accurately and timely within the EMR
• Participate in case conferences and team meetings

Lead & Support

• Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines
• Provide guidance and education to caregivers and support staff

What You Bring

• Graduate of an accredited Physical Therapy program (DPT or Master’s)
• Current PT license in the state of practice
• Minimum 1–2 years of experience (home health experience preferred)
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Current CPR certification
• Valid driver’s license, reliable transportation, and auto insurance
