Job summary

Enhabit Home Health & Hospice in Arizona seeks a licensed Physical Therapist (PT) to provide in‑home therapy services and assess mobility and functional needs for patients.

You will use specialized programs to improve outcomes, collaborate with care teams, and support independence in daily living.

Active PT license in the state and a valid driver’s license are required; one year of clinical experience is preferred.

Qualifications

• Must be licensed as a Physical Therapist (PT) in the state of employment.
• At least 1 year of clinical experience preferred.
• Demonstrate knowledge and skill in current therapy practice.

Responsibilities

• Examine and treat patients to improve mobility and daily living using physical modalities.
• Utilize specialty programs such as Balance and Fall Prevention or Joint Replacement programs to treat patients.
• Collaborate with care teams to support patient independence and outcomes.

Responsibilities

Our Physical Therapists (PTs) examine and treat patients with physical impairments through the use of physical modalities. The goal is to assist persons who are physically challenged to improve mobility and function, independent self‑care, other skills necessary for functioning in daily living. Our Physical Therapists (PTs) use our Specialty Programs, including Balance and Fall Prevention, Spine Safety, Total Hip and Knee Replacement Program, and our Total Shoulder Replacement Program to treat our patients.

Qualifications

• Be currently licensed as a Physical Therapist (PT) in the state of employment
• A minimum of one year of clinical experience is preferred
• Demonstrate knowledge and skill in current therapy practice
• Possess a valid state driver’s license and automobile liability insurance
