Job summary

VitalCaring Group is seeking a Home Health Physical Therapist (PRN) in Austin, TX. You’ll deliver patient-centered care in patients’ homes, assess function, and design individualized plans in collaboration with physicians and the care team.

You will supervise PTAs as needed and educate families while documenting progress in the EMR. Must have a PT degree, state license, CPR, and 1–2 years home health experience.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of home health experience preferred.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehab potential.
• Develop and implement individualized care plans in collaboration with the care team.
• Provide hands-on therapy to improve mobility, strength, balance and independence.
• Educate patients and caregivers on treatment plans, exercises and safety.
• Monitor patient progress and adjust treatment plans as needed.
• Document care and progress in the EMR and participate in team meetings.
• Supervise Physical Therapist Assistants (PTAs) when required.

What You’ll Do

Deliver Patient-Centered Care

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential
• Develop and implement individualized care plans in collaboration with physicians and the care team
• Provide hands‑on therapy to improve mobility, strength, balance, and independence
• Educate patients and caregivers on treatment plans, exercises, and safety strategies

Drive Outcomes & Recovery

• Monitor patient progress and adjust treatment plans as needed
• Support safe transitions of care and contribute to discharge planning
• Identify additional patient needs and collaborate with interdisciplinary teams
• Partner closely with nurses, therapists, and physicians to ensure coordinated care
• Document patient care and progress accurately and timely within the EMR
• Participate in case conferences and team meetings

Lead & Support

• Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines
• Provide guidance and education to caregivers and support staff

What You Bring

• Graduate of an accredited Physical Therapy program (DPT or Master’s)
• Current PT license in the state of practice
• Minimum 1–2 years of experience (home health experience preferred)
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Current CPR certification
• Valid driver’s license, reliable transportation, and auto insurance
