Job summary

ABL Health Care LLC is seeking a compassionate Physical Therapist (PT) to join our Home Health team in Napa, CA. This role offers full-time or part-time opportunities, with remote and in-field coverage to support patients in their homes.

The PT will evaluate patients, develop individualized treatment plans, and work closely with the interdisciplinary team to improve mobility, strength, and quality of life. Strong organizational and teamwork skills are essential.

Qualifications

• Current Physical Therapist (PT) license to practice in California.
• 1 year of home health or hospice care experience.
• Valid California driver’s license and ability to travel within a 15–25 mile radius regularly.

Responsibilities

• Provide physical therapy services within the scope of care, following the patient’s individualized plan, in coordination with the Interdisciplinary Team.
• Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change in patient condition to Physician, Case Manager, and/or Director of Nursing.
• Maintain accurate clinical records and submit documentation using a company-provided device.
• Comply with all company policies and procedures to ensure high standards of care.

Position Overview:

We are seeking an experienced, self-driven Full-Time / Part-Time Physical Therapist (PT) for Health Care to join our dedicated team and provide exceptional care throughout the region. As a PT, you will evaluate patients’ physical health, develop individualized treatment plans, and work to improve their mobility, strength, and overall quality of life. You will collaborate closely with our Clinical Director and Clinical Staff, ensuring the highest standards of care while providing support both remotely and independently in the field.
