Job summary

Enhabit Home Health & Hospice is seeking a licensed Physical Therapist to provide in-home care to patients across the Baker City area. You will evaluate, treat, and educate patients and families to improve mobility and function, following physician orders and the established care plan.

The role involves travel to patient homes, collaboration with a multi-disciplinary team, and participation in specialty programs such as Balance and Fall Prevention.

Qualifications

• Valid state PT license.
• Reliable transportation with driver’s license.
• Ability to travel to patient homes.
• Comfort with driving in all weather conditions.

Responsibilities

• Examine and treat patients to improve mobility and daily living activities.
• Provide skilled in-home physical therapy per physician orders and plan of care.
• Teach families and health care team members as part of care.
• Work with specialty programs like Balance and Fall Prevention, Spine Safety, hip/knee replacement, and total shoulder replacement.

Responsibilities

Our Physical Therapists examine and treat patients with physical impairments through the use of physical modalities. The goal is to assist persons who are physically challenged to improve mobility and function, independent self-care, other skills necessary for functioning in daily living. We have Specialty Programs that include Balance and Fall Prevention, Spine Safety, Total Hip and Knee Replacement Program, and our Total Shoulder Replacement Program. He/she administers skilled care to clients requiring intermittent professional services and teaches the family and other members of the health care team. These services are performed in accordance with the physician’s orders and the established plan of care, under the direction and supervision of the Branch Director.

Qualifications

1. Must possess a valid state driver’s license.
2. Must possess automobile liability insurance.
3. Must wear seatbelts at all times while driving.
4. Must have dependable transportation kept in good working condition.
5. Must be able to drive an automobile in all types of weather conditions.
6. Must be currently licensed in the state of employment.
