Job summary

AccentCare is seeking a Physical Therapist for Home Health in the Sacramento area. This full-time role covers Sacramento and surrounding areas, with a $20K sign-on bonus based on experience.

The PT will evaluate and treat patients, work within an interdisciplinary team, educate patients and families, document care, and maintain standards of practice. Licensing, CPR, and driving to locations are required.

Qualifications

• Master's degree in physical therapy from a CAPTE-accredited program.
• Minimum nine months of PT experience.
• Preferred community/home health experience.
• Licensed to practice as a PT in the state of operation.
• Valid CPR certification required.
• Must be a licensed driver and able to travel to all business locations.

Responsibilities

• Provides patient care and documents treatment per physician orders and agency policy.
• Performs patient evaluations and selects appropriate diagnostic materials.
• Notifies physician of assessment results and recommended interventions.
• Develops and documents treatment plans and priorities.
• Educates and counsels patients and families.
• Presents patient status and treatment plans to physicians and team members.
• Delivers treatments within patient needs and payer guidelines.
• Sets objective, measurable goals in collaboration with the team and patient.
• Meets productivity standards and uses work hours efficiently.

What You Need to Know

The Physical Therapist is responsible for providing evaluation and treatment to patients, functioning as a member of the interdisciplinary team, providing patient and family education and counseling regarding concerns, planning, implementing and modifying treatment procedures, maintaining appropriate documentation according to policies and procedures and current standards of practice. This position manages a caseload in accordance with agency productivity standards, attends and participates in patient and other conferences and agency in-services as assigned.

If you meet these qualifications, we would love to meet you:

• Master's degree in physical therapy approved by CAPTE accredited physical therapist education program
• Minimum nine months experience as a physical therapist

• Must possess and maintain valid CPR certification while employed in a clinical role

Responsibilities:

As a Physical Therapist, you will:

• Provides patient care and documents treatment in accordance with physician orders, agency policy and patient needs.
• Performs patient evaluations in accordance with agency policy and procedure. Selects appropriate diagnostic material to assess all aspects of patient care.
• Informs the physician of the results of the assessment and recommended clinical interventions prior to initiating care.
• Develops and documents appropriate treatment plans for patients including selecting appropriate intervention approaches, strategies, materials and establishing treatment priorities.
• Provides an organized and efficient patient treatment which includes advising, educating and counseling the patient.
• Presents pertinent patient information to physicians and keeps team members informed of patient status and treatment plan.
• Provides appropriate treatment to patients according to patient needs and within the parameters set by the agency and payer.
• Determines appropriate goals, which are objective and measurable in nature, in collaboration with team members and patients.
• Meets agency productivity standards and uses work hours efficiently.
