Nealy Pierce is seeking a Home Health Physical Therapist for a hospital-affiliated, non-profit system. The role offers PSLF eligibility, a base salary of $100,000+ and a pay-per-visit incentive, plus a $5,000 relocation bonus and travel mileage reimbursement.

The position includes PTO, comprehensive benefits, and retirement matching, with a weekend on-call rotation and a dedicated scheduling team. The ideal candidate has an accredited PT degree and at least 1 year of inpatient or home health

• Bachelor's, Master's, or Doctorate of Physical Therapy from an accredited program
• Minimum 1-year of inpatient or home health experience required
• Ability to independently manage your schedule and a metric-driven caseload

Responsabilidades

• Provide patient-centered physical therapy and education to help patients understand injuries and treatment
• Manage a metric-driven caseload across post-surgical, total joints, COPD, debility and general medical cases
• Contribute to planning and coordination with the care team

Home Health Physical Therapist - Hospital-Affiliated | PSLF Eligible | $100K+ Base

Highlights/Benefits

• Hospital-Affiliated Home Health (Non-Profit System)

• Compensation: $100,000+ Base Salary (Based on experience)
• Pay-Per-Visit (PPV) bonus structure: Earn additional premium pay for any visits completed over the 30-unit weekly target
• $5,000 relocation bonus
• Standard $0.63/mile reimbursement for travel (excluding first and last drive)
• Schedule Control: Dedicated team schedules your start-of-cares, allowing you to route the rest of your day efficiently
• Caseload: Post-surgical, total joints, COPD, debility, and general medical
• Weekend on-call rotation (1 weekend per month)
• Paid Time Off (PTO) = 4 weeks
