Job summary

Mission Healthcare is seeking a licensed Physical Therapist to join our team in a hybrid role serving regions in the West Valley, including Peoria. You will evaluate patients, develop personalized treatment plans, and implement therapeutic exercises to improve mobility and reduce pain.

Responsibilities include patient education, collaboration with an interdisciplinary team, and thorough documentation. A Master’s or Doctorate in Physical Therapy is required, with 1+ year home health experience

Qualifications

• Active Physical Therapist license in practicing state.
• Current BLS and CPR certification.
• Valid driver’s license required.

Pay range: $40-$40, depending on experience

SOC: $140-$150
Recert: $120-$130
Revisit: $90
Eval: $120-$130
Hourly: $35-$40

Schedule/Shift : Mon-Fri 8am-5pm

Territory/Location : West valley: Glendale, Central Phoenix, Peoria, Surprise and Sun City

Responsibilities:

• Educate patients and caregivers on exercises, safety precautions, and self-care strategies.
• Collaborate with an interdisciplinary team to provide holistic, patient-centered care.
• Maintain accurate and timely documentation of patient progress and treatment plans.

Qualifications:

• Active Physical Therapist licensein practicing state.
• Master’s or Doctorate degreein Physical Therapy.
• Minimum of 1 year of experienceas a Physical Therapist, home health experience strongly preferred.
• Current BLS, hands-on CPR certification.
• Valid driver’s license and reliable, insured vehicle.
