Job summary

AccentCare is seeking a Physical Therapist for home health to evaluate, treat, and educate patients in the home setting. You will work within an interdisciplinary team, document care, and adjust plans based on patient progress.

Qualified candidates hold a CAPTE-approved PT degree, state licensure, CPR, and a valid driver's license with the ability to travel to multiple locations. Prior home health experience is preferred. This position offers a meaningful caseload with competitive compensation.

Qualifications

• Master's degree in physical therapy approved by CAPTE-accredited program.
• Minimum nine months experience as a physical therapist.
• Preferred community/home health experience.
• Must be licensed to practice as a physical therapist in the state of operation.
• Must possess and maintain valid CPR certification while employed.
• Must be a licensed driver who can travel to all business locations.

Responsibilities

• Provide patient care and document treatment per physician orders and policy.
• Perform patient evaluations and select appropriate diagnostic material.
• Discuss assessment results with physician and initiate care.
• Develop and document treatment plans with priorities and methods.

The Physical Therapist is responsible for providing evaluation and treatment to patients, functioning as a member of the interdisciplinary team, providing patient and family education and counseling regarding concerns, planning, implementing and modifying treatment procedures, maintaining appropriate documentation according to policies and procedures and current standards of practice. This position manages a caseload in accordance with agency productivity standards, attends and participates in patient and other conferences and agency in-services as assigned.

Responsibilities:

As a Physical Therapist, you will:

• Provides patient care and documents treatment in accordance with physician orders, agency policy and patient needs.


• Performs patient evaluations in accordance with agency policy and procedure. Selects appropriate diagnostic material to assess all aspects of patient care.


• Informs the physician of the results of the assessment and recommended clinical interventions prior to initiating care.


• Develops and documents appropriate treatment plans for patients including selecting appropriate intervention approaches, strategies, materials and establishing treatment priorities.


• Provides an organized and efficient patient treatment which includes advising, educating and counseling the patient.


• Presents pertinent patient information to physicians and keeps team members informed of patient status and treatment plan.


• Provides appropriate treatment to patients according to patient needs and within the parameters set by the agency and payer.


• Determines appropriate goals, which are objective and measurable in nature, in collaboration with team members and patients.


• Meets agency productivity standards and uses work hours efficiently.
