As a PT, you will deliver 45 minutes of top-notch therapy, assess patients, develop personalized treatment plans, and help them achieve their goals with compassionate care and thorough documentation.

Qualifications

• Active California Physical Therapist (PT) license required.
• 1 year+ PT experience preferred.
• Strong self-management and independent work abilities.
• Excellent communication (verbal & written) and compassionate care.
• Proficient in documentation and timely completion.

Responsibilities

• Provide 45 minutes of rehabilitation services to patients in their homes.
• Assess patients' physical condition and develop personalized treatment plans.
• Deliver hands-on therapy and adjust plans as needed.
• Help patients achieve goals and improve quality of life.

Job Summary:

• Utilize your expertise to assess patients' physical condition

Qualifications:

• Minimum 1 year of licensed Physical Therapist experience.
• Strong self‑management skills and the ability to work independently.
• Responsive and timely communication through messaging.
• Proficient in documentation and ensuring timely completion.
• Excellent verbal and written communication skills with a compassionate approach.

Requirements:

• Active California Physical Therapist (PT) license.
• Valid driver's license, reliable transportation, and auto insurance.
• Current CPR certification and professional liability insurance.
• A physical examination was conducted within the last 2 years.
• Tuberculosis (TB) test conducted within the last 1-2 years depending on test type.
• Possession of an Employer Identification Number (EIN) with an active business license.
