Job summary

The Savoy at Fort Lauderdale Rehabilitation & Nursing Center is seeking a licensed Physical Therapist (PT) to join our care team. You will assess referred patients, develop goals, and implement treatment plans under physician direction.

You will supervise Physical Therapy Assistants, document progress, and communicate with patients and families. Experience in long-term care or geriatrics is preferred; Florida licensure is required. Punctuality and professional conduct are essential.

Qualifications

• Must be a graduate of an accredited Physical Therapy program.
• Must hold a valid Florida physical therapy license and registration.
• Oversee and supervise Physical Therapy Assistants.
• Experience with long-term care or geriatric care preferred.
• Strong documentation skills.
• Ability to complete evaluations, progress notes, recertifications and discharge summaries.
• Collaborates with multidisciplinary team to provide patient-centered care.
• Professionalism and timely attendance.
• Compliance with confidentiality, HIPAA, and Residents' Rights.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.

• Document and record patient condition and educate patients and families in physical therapy methods.
• Supervise Physical Therapy Assistants and coordinate with other disciplines.
• Attend to evaluations, weekly progress notes, recertifications, and discharge documentation.
• Collaborate with all disciplines to provide best patient-centered treatment and care.
• Demonstrate exceptional patient service and clinical excellence.
• Report to work on time and adhere to scheduled hours.
• Maintain confidentiality and adhere to HIPAA and Residents' Rights.

The Savoy at Fort Lauderdale Rehabilitation & Nursing Center is seeking a Physical Therapist (PT) to join our growing team.

Physical Therapist (PT) Duties Include:

The Physical Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Physical Therapist will establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician. As well, the Physical Therapist must set realistic and achievable goals for their patients, document and record the patients condition and educate patients and families in an appropriate physical therapy method.

Physical Therapist (PT) Requirements/Qualifications:

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid Florida professional license and registration.
• The Physical Therapist is responsible for overseeing and supervising Physical Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
