FeldCare Connects is seeking a driven Physical Therapist to provide high-quality home health care to patients in Auburn, CA and the surrounding areas as an independent contractor. You control your schedule and coverage areas, earning per-visit pay with no minimums, while our EMR and Cliniconnects app streamline documentation and patient invitations.

Responsibilities

• Provide one-on-one home physical therapy to patients in Auburn and surrounding areas.
• Observe and document patient responses and progress during treatment.
• Determine and adjust the plan of care based on patient needs.
• Utilize Cliniconnects app to accept or decline patients and coordinate visits.

FeldCare Connects, an app-based referral network, is seeking an enthusiastic, self-motivated Physical Therapist to deliver quality home healthcare to patients of all ages.

Independent Contractor to serve Auburn, CA and the surrounding areas.

Qualifications:

• Ability and enthusiasm in working with all ages
• Current Physical Therapist license issued by the state Board of Physical Therapy
• The required license MUST be current, active, and unrestricted; a conditional, provisional, or restricted license will NOT be accepted. Limited permits will not be accepted.
• Completion of an accredited Physical Therapy program
