Qualifications

• Bachelor’s degree or higher in Physical Therapy from an accredited program.
• Active, unencumbered license to practice in the state of employment.
• Basic computer skills and ability to navigate service delivery and EHR software.

Responsibilities

• Restore patients to optimal physical function within a SNF setting and support goals with patients, physicians, and therapy teams.
• Educate patients on maintenance of function after discharge and provide independent exercise guidance.
