Job summary

Choice Health at Home in Boulder, CO is seeking a Physical Therapist to deliver comprehensive home health services. You will evaluate patients, create and implement personalized treatment plans, and document progress while collaborating with the care team.

Requirements include an accredited PT program and state licensure, CPR certification, and a valid driver’s license. A minimum of 1 year home health experience is preferred. This is a full-time role with a $5,000 sign-on bonus.

Responsibilities

As a Physical Therapist , you will:

Perform and document an initial exam , evaluating data to identify problems and determine a diagnosis prior to intervention.

Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.

Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.

Applies concepts of infection control/universal precautions in performing patient care

Assure continuity of quality patient care delivered with appropriate documentation

Provide effective communication to patient/family, team members and other health care professionals

Promote agency philosophy and administrative policies

Maintain and complete all documentation within company stated guidelines.

Requirements

Successful completion of accredited physical therapy program and state licensure exam.

Working Hours

To meet the needs of our patients and clients, employee work hours may vary and will be set by each employee’s supervisor, manager, and operating license documentation.

Employees should consult with their supervisors regarding their schedules and time recording procedures. Choice Health at Home, the Company, is open Monday through Friday, office hours

are 8:00 am to 5:00 pm (CST).

Mileage

The Company recognizes the use of personal vehicles by personnel for business purposes as part of their employment. The company will reimburse the eligible employee $0.50 per

mile to cover personal care use, auto insurance, depreciation, and gas. Each employee who is eligible for mileage reimbursement from the Company will be required to track and report mileage

accurately and timely each pay period.
