Job summary

Nurse Next Door is seeking a licensed Physical Therapist to provide in-home care in Virginia. The role focuses on evaluating mobility and developing customized therapy plans to improve safety and independence for private clients.

You will collaborate with interdisciplinary teams, monitor progress, and educate clients and families on home exercise programs while maintaining thorough documentation. A valid VA PT license and 2+ years in home health are preferred, with BLS and background checks as

Qualifications

• Current Virginia PT license in good standing.
• Minimum 2 years of experience, preferably in home health.
• Current BLS certification preferred; TB screening and criminal background check.
• Legal authorization to work in the United States; valid driver's license and reliable transportation.

Responsibilities

• Perform initial and ongoing in-home physical therapy evaluations to assess mobility, strength, balance and function.
• Develop and implement customized therapy treatment plans focused on safety, independence and rehabilitation goals.
• Provide interventions such as therapeutic exercise, gait and mobility training and fall prevention techniques.
• Educate clients and families on home exercise programs and assistive device use; monitor progress and adjust plans as needed.
• Collaborate with RNs, physicians, and case managers; maintain accurate documentation using electronic systems.

Responsibilities and Duties:

• Perform initial and ongoing in-home physical therapy evaluations to determine client mobility, strength, balance, and functional ability.
  
• Develop and implement customized therapy treatment plans focused on safety, independence, and rehabilitation goals.
  
• Provide therapy interventions such as:
  
• Therapeutic exercise
  
• Gait and mobility training
  
• Balance and fall prevention techniques
  
• Educate clients and family members on home exercise programs and assistive device use.
  
• Monitor and document progress toward therapy goals and modify plans as needed based on clinical outcomes.
  
• Collaborate with interdisciplinary care teams including RNs, physicians, and case managers.
  
• Maintain accurate and timely documentation using electronic systems.
  
• Promote functional independence and prevent complications through patient-centered care.

Requirements:

• A current Virginia Physical Therapist (PT) license in good standing.
  
• Minimum of 2 years of experience, preferably in home health.
  
• Valid TB screening and criminal background check.
  
• Current BLS Certification (preferred).
  
• Strong verbal and written communication skills in English; additional languages are a plus.
  
• Reliable transportation and a valid driver’s license.
  
• Legal authorization to work in the United States.
  
• Availability during morning hours is required.
