Job summary

Nassau Rehabilitation and Nursing Center is actively seeking a dedicated Physical Therapist to join our interdisciplinary rehabilitation team in Hempstead. This full-time PT position offers the chance to impact patient recovery and quality of life in a skilled nursing setting.

The ideal candidate holds a Doctor of Physical Therapy, active licensure, and 2+ years of experience, preferably in nursing homes. We value compassion, strong communication, and collaborative care with an integrated team.

Qualifications

• Doctor of Physical Therapy (DPT) from an accredited program.
• Active state licensure as a Physical Therapist.
• Minimum of 2 years of experience preferred.
• Nursing home or skilled nursing experience preferred.

Responsibilities

• Consult with patients to evaluate physical conditions, symptoms, and medical history.
• Diagnose movement dysfunction through observation and assessment.
• Develop and implement individualized treatment plans.
• Coach patients in therapeutic exercises and stretches.
• Provide hands‑on therapy (e.g., massage) to relieve pain and promote healing.
• Monitor progress, adjust treatment plans, and maintain accurate patient records.
• Educate patients and families on in‑home care and recovery expectations.
