Position Overview: The Physical Therapist provides direct rehabilitation care to patients in a skilled nursing facility, focusing on improving functional outcomes and quality of life within a patient-centered environment.

Key Responsibilities

• Develop, assess, and adjust individualized treatment plans based on patient needs and goals.
• Supervise and evaluate therapy team members, including PTAs and students.
• Accurately document patient progress and utilize therapy software effectively.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
• Assist in emergency procedures and ensure safety, sanitation, and patient rights.
• Maintain confidentiality, adhere to legal and organisational policies, and demonstrate professionalism.
• Participate in community health projects and support departmental goals.

Qualifications & Skills

• Graduate from an accredited physical therapy program (BSPT, MSPT, or DPT). Must be licensed/registered in the applicable state and maintain an active license. >1 year of experience preferred; new grads are welcome. >CPR certification required, obtained during orientation if not already certified.
• Proficient in Microsoft Word, Excel, email, and therapy documentation software.
• Excellent communication, body mechanics, and decision-making skills.
• Ability to work independently and as part of a team, demonstrating professionalism and integrity.

Essential Functions

• Implement and monitor treatment plans, establishing measurable goals.
• Evaluate and oversee care provided by therapy staff and students.
• Maintain accurate charts and documentation.
• Exhibit dependable attendance and excellent customer service.
• Perform physical activities including sitting, standing, lifting, and mobility tasks as required.
