Job summary

Creative Health Solutions is seeking a skilled Physical Therapist to join our team at Skilled Nursing facilities. You will develop and administer therapy programs to improve patients' physical and overall well-being, working closely with a multidisciplinary team.

You will document patient progress, communicate treatment plans to other healthcare professionals, and stay current with therapy techniques. A valid state PT license is required to practice in Indiana.
