Job summary

Creative Health Solutions is seeking a skilled Physical Therapist to join our weekend rotation team at Skilled Nursing facilities. The role focuses on developing and administering therapy programs to improve patients' physical, mental, and emotional well-being, working closely with a multidisciplinary team.

You will document patient progress, communicate with other healthcare professionals, educate patients and families on procedures, and stay current with therapy techniques.

Qualifications

• Current Physical Therapist license in the state.
• Experience in skilled nursing facilities preferred.

Responsibilities

• Develop and administer therapy programs aimed at improving patients' physical, mental, and emotional well-being.
• Work closely with a team of healthcare professionals to create personalized treatment plans for patients.
• Document patient progress and communicate with healthcare professionals regarding treatment plans and patient outcomes.
• Continuously educate patients and their families on therapeutic procedures and techniques.
• Stay up to date with the latest therapy techniques and methodologies.
