Job summary

Creative Health Solutions is seeking a PRN Physical Therapist to join our growing team. We provide therapy services at Skilled Nursing facilities since 2001, focused on developing great clinicians and promoting autonomy in clinical decision-making.

You will develop and administer therapy programs with a team of healthcare professionals, document patient progress, educate patients and families, and stay updated with current therapy techniques. Valid State PT License required.
