Franklin Restorative Care Center in Minnesota seeks a licensed Physical Therapist to evaluate, plan, and implement rehabilitation services for residents, aiming to restore function, improve mobility, relieve pain, and promote independence.
You will work with an interdisciplinary team to develop individualized treatment plans, supervise PTAs, document progress, and ensure safety and compliance while helping residents achieve meaningful quality of life.
The Physical Therapist (PT) is responsible for evaluating, planning, and implementing physical therapy services to restore function, improve mobility, relieve pain, and prevent or limit permanent disability. The PT collaborates with the interdisciplinary team to ensure individualized, resident-centered care that promotes health and independence.
Evaluate patients' physical abilities, needs, and goals through assessments and screenings
Develop individualized treatment plans in accordance with physician orders and clinical best practices
Provide direct physical therapy treatments including therapeutic exercise, gait training, balance, and mobility interventions
Train patients, families, and caregivers on safe mobility, home exercise programs, and use of assistive devices
Monitor and document patient progress, updating treatment plans as needed to maximize outcomes
Collaborate with physicians, nursing, and other therapy professionals to ensure coordinated care
Maintain compliance with federal, state, and professional practice standards
Supervise and guide Physical Therapy Assistants (PTAs) as appropriate
Current and active Physical Therapist (PT) license in Minnesota
Graduate of an accredited physical therapy program
Strong clinical skills with knowledge of rehabilitation techniques, equipment, and safety standards
Excellent communication, documentation, and interpersonal skills
Ability to work effectively in a team environment
Prior experience in skilled nursing, rehabilitation, or long-term care setting preferred
