Job summary

Lifepoint Health's St. Luke's Health-Brazosport Hospital seeks a Physical Therapist (PT) to join as a PRN team member. You will evaluate patients, develop treatment plans, educate nursing staff and families, and document care to support recovery and safety.

Responsibilities include communicating progress to the supervisor, assisting with scheduling and post charges, and maintaining equipment and a clean work area, while adhering to regulatory standards.

Qualifications

• Degree from an accredited Physical Therapy program.
• Current and unrestricted Physical Therapy license in the state of services.
• Current CPR certification.
• Strong organizational and communication skills.

• Evaluate a patient's condition, develop a treatment plan and help them improve day by day.
• Instruct nursing staff and patients' families on follow-through programs.
• Communicate patient progress to supervisor and team; assist with scheduling and post charges.
• Document patient care in accordance with regulatory and accrediting requirements.
• Maintain equipment and a safe, clean work area.
• Perform duties in line with professional standards and company policy.
• Other duties as assigned.

How you'll contribute

A Physical Therapist (PT) who excels in this role:

• Put your physical therapy skills to work where they're really needed - evaluate a patient's condition, develop a treatment plan, and help them get better, day by day. You'll also instruct the nursing staff and the patient's families on follow-through programs that build on the progress they've made
• Communicate patient progress or problems to supervisor and other team members; assist with patient scheduling and post charges daily to patient records
• Document patient care in accordance with regulatory, licensing, payer and accrediting requirements
• Instruct patient's family or nursing staff in follow-through programs
• Maintain equipment and work area in a safe and clean condition
• Handle job responsibilities in accordance with the Company's Code of Business Conduct, the Corporate Compliance Agreement, appropriate professional standard and applicable state/federal laws
• Other duties as assigned

What we're looking for

Applicants should have a Degree from an accredited Physical Therapy Program. Additional requirements include:
