Job summary

Highland Care Center in Utah is seeking a licensed Physical Therapist (PT) for per diem coverage to support residents’ mobility and well-being. You will assess patients, design personalized treatment plans, and collaborate with an interdisciplinary team to optimize outcomes.

The ideal candidate holds a Doctor of Physical Therapy (DPT) and Utah licensure, and is available to work variable shifts including weekends and holidays.

Qualifications

• Graduation from an accredited Physical Therapy program.
• Current Utah Physical Therapist licensure.
• Ability to work variable schedule including weekends/holidays on a per diem basis.

Responsibilities

• Perform comprehensive patient evaluations and develop individualized treatment plans.
• Implement evidence-based PT interventions and modalities.
• Collaborate with nurses, therapists, and other staff to ensure cohesive care.
• Monitor progress and adjust plans as needed and communicate with patients and families.
• Participate in quality improvement initiatives to enhance outcomes.

About the Role:

As a Physical Therapist (PT) at Highland Care Center, you will have the opportunity to work with a diverse patient population and collaborate with our interdisciplinary team to develop and implement individualized care plans. Your expertise and compassion will enable you to empower our residents to achieve their maximum potential, promoting physical function, mobility, and overall well-being.

Responsibilities:

• Conduct comprehensive patient evaluations and develop personalized treatment plans
• Implement evidence-based physical therapy interventions, including exercise programs, manual therapy, and modalities
• Collaborate with nurses, occupational therapists, and other healthcare professionals to ensure interdisciplinary care
• Monitor patient progress, adjust treatment plans as needed, and communicate effectively with patients and families
• Participate in quality improvement initiatives to optimize patient outcomes and satisfaction

Requirements:

• Graduate degree in Physical Therapy from an accredited institution
• Current licensure as a Physical Therapist in the state of Utah
• Ability to work a variable schedule, including weekends and holidays, on a per diem basis
