HealthPro Heritage in River Forest, IL is seeking a Part-time Physical Therapist to assess and treat seniors in an Assisted Living Community. The role involves evaluating mobility and pain, prescribing and guiding therapeutic exercises, and educating patients and families to support home routines.

You will document progress, collaborate with a multidisciplinary team, and contribute to high-quality care while enjoying a flexible schedule and a mission-driven environment.

Qualifications

• Degree in Physical Therapy from an accredited institution.
• Valid state licensure as a Physical Therapist, or license eligible.
• Strong clinical assessment and diagnostic skills.
• Proficient in developing and implementing effective treatment plans.
• Excellent communication and interpersonal skills with a patient-centered approach.

Responsibilities

• Patient Assessment: Evaluate patients' physical conditions, including mobility, strength, and flexibility.
• Treatment and Education: Provide therapeutic exercises, manual therapy, and other treatments; educate patients and families on exercises, posture, and home routines.
• Documentation: Maintain accurate records of evaluations, progress, and treatment plans, ensuring compliance with legal and ethical standards.
• Collaboration: Work with healthcare providers to ensure effective patient care and participate in multidisciplinary team meetings as needed.
