Adaptive Home Health is seeking a Physical Therapist to provide high-acuity, patient-centered care in patient's homes. The role requires conducting comprehensive assessments, developing care plans, and maintaining communication with physicians and care teams.

Qualifications

• Current Texas Physical Therapist (PT) license required.
• One year of physical therapy experience required.
• Experience with Homecare Homebase is helpful, but not required.
• Previous Home Health experience is helpful, but not required.
• Current Texas driver's license is required.
• BLS Certification is required.

Responsibilities

• Evaluate patients and perform appropriate therapy interventions.
• Develop a comprehensive plan of care and coordinate multi-disciplinary care.
• Perform prompt clinical documentation and visit notes.
• Continued communication with physician, care team members, patient and family.
• Implement and maintain physical therapy protocols.
• Manage patient treatment and outcomes directly, or through PTA supervision.

Job Description

Physical Therapist is responsible for comprehensive assessments in the patient's home, including the completion of skilled documentation through Homecare Homebase.

• Evaluate patients and perform appropriate therapy interventions
• Develop a comprehensive plan of care and coordinate multi-disciplinary care
• Perform prompt clinical documentation and visit notes
• Continued communication with physician, care team members, patient and family
• Comfortable working in all aspects of the home health setting
• Implement and maintain physical therapy protocols
• Manage patient treatment and outcomes directly, or through PTA supervision

Requirements

• Current Texas Physical Therapist (PT) license
• One year of physical therapy experience
