Job summary

Trilogy Health Services, LLC is seeking a Physical Therapist to provide comprehensive physical therapy services at The Oaks at Byron Center, US-MI Byron Center. The PT will assess, plan, and implement therapy in coordination with an interdisciplinary team.

Qualifications

• Degree in Physical Therapy from an accredited program.
• Current CPR certification preferred.
• 0–1 year of experience in physical therapy is acceptable.

Responsibilities

• Conduct assessments to evaluate patients' physical conditions and rehabilitation needs.
• Develop and implement personalized treatment plans with exercises, manual therapy, and modalities.
• Educate patients and families on exercise techniques, posture, and body mechanics.
• Maintain accurate documentation of patient progress and treatment plans.
• Collaborate with interdisciplinary team to ensure comprehensive care.
• Other duties as assigned.

POSITION OVERVIEW

Job Summary The Physical Therapist (PT) is responsible for providing a full range of physical therapy services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the position's qualifications, professional practices and ethical standards.

Roles and Responsibilities

• Conduct thorough assessments to evaluate patients' physical conditions, functional limitations, and rehabilitation needs.
• Develop and implement personalized treatment plans, including therapeutic exercises, manual therapy techniques, and modalities to improve patients' physical function.
• Educate patients and their families on exercise techniques, posture, body mechanics, and strategies to manage their condition effectively.
• Maintain accurate and up-to-date documentation of patient progress, treatment plans, and outcomes in compliance with facility policies and regulatory requirements.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.
