Job summary

Saber Health Group is seeking a Physical Therapist for a Skilled Nursing Facility in Marion, NC. The role focuses on returning patients to optimal function and achieving therapy goals within a SNF setting.

You will educate patients for discharge, guide independent exercise, and promote activity modification to sustain gains. A bachelor’s degree in PT and an active license are required.

Qualifications

• Bachelor’s degree or higher in Physical Therapy from an accredited college or university.
• Must maintain an active, unencumbered license granted from state of employment.
• Must possess basic computer skills and ability to navigate through various software programs, including service delivery software and the electronic health record software.

Responsibilities

• Return patient to optimal physical function and facilitate achievement of goals in a SNF setting.
• Instruct the patient in maintaining achieved level of function after discharge through education and independent exercise.
• Educate patients regarding activity modification and discharge planning.
