Job summary

Synchrony Rehab seeks a licensed Physical Therapist to deliver comprehensive PT services including assessment, treatment planning, and therapeutic interventions in a collaborative setting.

The role emphasizes patient and family education, accurate documentation, and coordination with an interdisciplinary team to optimize rehabilitation outcomes.

Qualifications

• Mastery of PT assessment, plan of care, and interventions in a team setting.
• Ability to educate patients and families on exercises and safe movement.
• Maintain compliant documentation and progress notes per policy.

Responsibilities

• Perform comprehensive PT assessments and determine rehab needs.
• Develop and implement individualized treatment plans with exercises, manual therapy, and modalities.
• Educate patients and families on postural strategies and home exercise.
• Maintain accurate progress notes and outcomes in accordance with regulations.
• Collaborate with interdisciplinary team to coordinate care.
• Other duties as assigned.

Qualifications

The Physical Therapist (PT) is responsible for providing a full range of physical therapy services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the position’s qualifications, professional practices and ethical standards.

• Conduct thorough assessments to evaluate patients’ physical conditions, functional limitations, and rehabilitation needs.
• Develop and implement personalized treatment plans, including therapeutic exercises, manual therapy techniques, and modalities to improve patients’ physical function.
• Educate patients and their families on exercise techniques, posture, body mechanics, and strategies to manage their condition effectively.
• Maintain accurate and up-to-date documentation of patient progress, treatment plans, and outcomes in compliance with facility policies and regulatory requirements.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.

Education: Master Degree

Experience: 0-1 years

Licenses and Certifications

• Must have and maintain a current, valid state PT licensure or certification
• Current valid CPR certification, preferred

Travel: Yes. Minimally, as required (Typically less than 10%)
