Job summary

Adaptive Home Health is seeking a Physical Therapist to provide skilled home health therapy in a field-based role. You will own patient caseloads, perform in-home assessments, and deliver PT visits to improve function and safety in the patient’s home.

You will develop and execute plans of care, coordinate with the care team, and document thoroughly using HCHB. A Texas PT license and ability to travel daily are essential for success.

Qualifications

• Active Texas PT license (in good standing).
• Strong clinical judgment and ability to work independently in the field.
• Comfortable escalating changes in condition to RN/MD with clear documentation.
• Excellent communication and documentation skills.
• Valid driver’s license and ability to travel daily.
• Safety-focused mobility skills: transfers, gait, balance.
• Competency in core home health PT scope.
• Ability to coach patients and caregivers with home programs.

Responsibilities

• Own the caseload as a case manager: therapy plan, progression, and outcomes.
• Deliver comprehensive in-home PT visits and assessments.
• Build a plan of care: gait/balance, transfers, strengthening, functional mobility, home safety.
• Set measurable goals and monitor progress.
• Coordinate with care team to align orders and updates.
• Supervise PTAs where appropriate and manage outcomes.
• Coach patients and caregivers with practical home programs.
• Document promptly and thoroughly in HCHB.

Physical Therapist (PT) - Home Health

• Schedule: Full-time Monday–Friday + occasional weekends (as needed)

• Location: Field Based

• Compensation (Productivity Points): 28 PP

What you will do:

• Own the caseload as a case manager: you’re accountable for the patient’s therapy plan, progression, and outcomes — not just individual visits.

• Complete comprehensive in-home assessments and deliver skilled PT visits.

• Build and execute a clear plan of care: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.

• Set measurable goals, track objective progress, and adjust treatment quickly when patients stall.

• Drive multidisciplinary coordination: communicate changes in condition, align on orders, and keep the care team moving.

• Manage treatment and outcomes directly, including PTA supervision where appropriate.

• Coach patients and caregivers with practical home programs that actually get done.

• Document promptly and thoroughly in Homecare Homebase (HCHB) — defensible, efficient, and consistent.

Requirements

Must-haves

• Active Texas PT license (in good standing)

• Strong clinical judgment and comfort working independently in the field

• Comfortable escalating changes in condition to RN/MD and documenting rationale clearly

• Strong communication and documentation

• Reliable follow-through on orders, frequencies, and patient goals

• Current driver’s license and comfort with daily travel across assigned territory

• Able to carry/transport basic therapy equipment as needed

• Competency in core home health PT scope

  • Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations

  • Strengthening, functional mobility, and home safety assessment/modification education

  • Patient/caregiver coaching with practical home programs

Nice-to-haves

• Prior home health experience

• Homecare Homebase (HCHB) experience (or similar home health EMR)

• Deeper clinical experience in high-signal PT populations

• Experience supervising/mentoring PTAs and collaborating on care plans

• Strong organization and time management
