Qualifications

• Doctorate in Physical Therapy (DPT) from an accredited program.
• Meets qualifications as established by APTA.
• Active Physical Therapy License in State of Practice.
• Current BLS/CPR certification.
• Reliable transportation and a valid driver's license.
