Job summary

Palmetto Subacute Care Center in Miami, FL is seeking a Physical Therapist (PRN) for weekend coverage. Per diem shifts available with a daily pay option. The dedicated team emphasizes quality care for the frail elderly in a small, 95-bed facility.

The PT will assess referred patients, deliver patient-centered rehabilitation, document progress, and supervise PT Assistants, while complying with HIPAA and Residents' Rights. Equal opportunity employer.

Qualifications

• Graduate from an accredited Physical Therapy Assistant program.
• Valid FL license and registration as a PT/PTA.
• Provide comprehensive treatment under supervision of Physical Therapist.
• Timely weekly progress notes documentation.
• Collaborate with all disciplines for patient-centered care.
• Experience in geriatric rehab, neurology, cardiac or general rehab preferred.
• Maintain confidentiality per HIPAA and Residents' Rights.

Responsibilities Include:

The Physical Therapist is responsible for the assessment of referred patients that require rehabilitation services, provides patient treatment that meets patient needs, uses current treatment knowledge in accordance with clinical procedures. Writes accurate complete and clear documentation in accordance with regulatory, licensing, payor and accrediting requirements. Responsible for overseeing and supervising Physical Therapy Assistants.

Qualifications/Requirements:

• Must have graduated from an accredited Physical Therapy Assistant college or university.
• Must have a valid professional license and registration for state of FL
• Provide comprehensive treatment to patients under supervision of Physical Therapist
• Well versed with documentation writing
• Document patient’s progress on weekly progress note in a timely fashion.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Experience in geriatric rehabilitation, adult neurological, cardiac or general rehabilitation preferred
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
