Job summary

FOX Rehabilitation is expanding in St. Charles, MO, and seeks licensed Physical Therapists to deliver one-on-one care in private residences or Senior Living Communities. The Geriatric House Calls model emphasizes autonomy, flexible scheduling, and meaningful outcomes for older adults.

Clinicians may incorporate telehealth and other virtual care as part of schedules to improve continuity of care and reach more patients, while building strong patient relationships in the community FOX serves.

Qualifications

• PT license in the state(s) of practice or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency; CPR certification on current status.

Responsibilities

• Provide one-on-one physical therapy to older adults in their homes or communities.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support wellness, mobility, and independence.
• Document effectively in FOX’s EMR in line with regulatory standards.
• Collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care to extend access and flexibility.

Physical Requirements

FOX clinicians must be able to meet the physical demandsof in-homeandcommunity-basedcare, including:

• Exerting medium physical effort, including lifting 25–50lbsoccasionally, 10–25lbsfrequently, and up to 10lbsconstantly
• Performing CPR
• Providing moderate to maximumassistancewith patient transfers and bed mobility
• Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities
• Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model
• Wearing PPE whenrequired
