Qualifications

• Graduation from an accredited program with a DPT or equivalent.
• Holds current licensure/registration to practice physical therapy in NC.

Responsibilities

• Provide comprehensive PT evaluation based on physician orders.
• Document findings on standardized forms and adhere to guidelines.
• Develop and update treatment plans with goals, frequency, duration, and modalities.
• Ensure orders for evaluations, treatments, re-certifications and discharges are obtained.
• Deliver treatments using appropriate modalities and modify plans as needed.
• Document daily progress and weekly summaries accurately.
