A leading rehabilitation provider in Saint Joseph, Michigan is seeking an Entry-level Physical Therapist to join their team. The role involves evaluating rehabilitation patients, developing treatment plans, and working with a supportive staff in a dynamic environment. Qualifications

• Evaluate patients for rehabilitation services within 24 to 48 hours of physician’s orders.
• Develop effective treatment plans.
• Supervise the Licensed Physical Therapy Assistant.

Responsibilities

• Treat patients according to the treatment plan of care.
• Communicate with physicians, clinical managers, and other health team members.
• Participate in interdisciplinary team meetings.
• Supervise the Licensed Physical Therapy Assistant in delivering services.
• Communicate with physicians and health team members regarding patient progress.
• Participate in Plan of Care Meetings and Interdisciplinary Team Meetings.
• Meet or exceed productivity standards.
• Record notes and participate in daily billing per policy.
• Instruct families or facility staff in maintenance programs.
• Participate in discharge planning.
• Recommend equipment to facilitate care.
• Report equipment issues to facility staff.
• Act as a clinical supervisor for PT/PTA programs for area universities.

• Assist with special projects and other duties.

The Physical Therapist provides competent, courteous, and respectful evaluation and treatment of patients for difficulties with mobility, function, strength and positioning and duties outlined in APT scope of practice and respective licensure laws. The PT communicates and collaborates with families, physicians and other health team members and maintains impeccable, professional documentation of services in the medical record. The PT also conducts engaging in-services and training for facility staff on an ongoing basis in restorative and functional maintenance programs. The PT reports to the Therapy Program Manager and ultimately to the Director of Operations. CRI’s PTs model and promote competent, courteous and professional services to internal and external customers to develop and maintain a competitive advantage. CRI PTs maintain career development through continuing education and training opportunities and networking. Requirements

Essential Functions of the Job:

• Evaluate patients for rehabilitation services within 24 to 48 hours of physician’s orders.
• Develop effective treatment plans.
• Treat patients according to the treatment plan of care.
• Supervise the Licensed Physical Therapy Assistant in the delivery of clinical services and interactions with other staff, patients and family members.
• Communicate with physicians, clinical managers, and other health team members, patients, family and facility staff regarding patient progress, problems and plans.
• Participate in Plan of Care Meetings upon request and Interdisciplinary Team Meetings.
• Achieve or exceed the minimum productivity standard established by CRI.
• Participate in in-service training programs for other staff in the facility.
• Record treatment notes, progress notes, provide objective input for discharge summaries, and complete daily billing of services per policy and procedure.
• Instruct patient’s families and/or facility staff in maintenance programs.
• Participate in discharge planning.
• Recommend any needed equipment to the appropriate personnel which would serve to facilitate improved delivery of clinical service.
• Report any problems with department equipment to the facility personnel.
• Act as a clinical supervisor for affiliating Physical Therapy and Physical Therapy Assistant programs for area universities and colleges.
• Consistently portray the mission, vision, core values, cornerstones and professional image of Concept Rehab, exercise good judgment in the performance of the job.
• Special projects and other duties as assigned.
