Job summary

Park Place Care in Georgetown, TX, is seeking a Per Diem Physical Therapist to join our in-house rehab team. This role focuses on person-centered care to achieve positive outcomes for residents.

You will consult with patients, assess tests, develop plans, implement personalized rehab programs, and document thoroughly while coordinating with the IDT and attending Rehab Director meetings.

Qualifications

• Must hold an active Texas Physical Therapy license in good standing.
• Experience in SNF/long-term care settings preferred.
• Familiarity with Net Health Electronic Documentation System preferred.

Responsibilities

• Consult with patients to learn about their physical condition.
• Assess evaluations and test results.
• Develop and implement treatment plans using various techniques.
• Create personalized rehabilitation programs.
• Administer PT treatments to relieve pain and restore function.
• Ensure all treatment aligns with the plan of care.
• Monitor patient progress and adjust approach as needed with IDT.
• Provide education to patients and families.
• Collaborate with the interdisciplinary team to ensure quality care.
• Complete documentation in a timely, compliant manner.
• Attend Rehab Director meetings.

Wehave an amazing opportunity for a Per Diem (PRN) Physical Therapist!

We are looking for a PT to join our in-house team of outstanding professionals that service our residents through person-centered care to achieve positive outcomes!

Physical Therapist Responsibilities:

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with other members of the interdisciplinary team to ensure quality patient care is provided.
• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.
• Attend required meetings as designated by the Director of Rehab.

Physical Therapist Skills:

• SNF/Long term care experience preferred.
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

Physical Therapist Requirements:

• Active/Valid Texas license in Physical Therapy
• In good standing with all regulatory agencies and licensing boards
