  • Screen residents to determine PT needs and modify plans as indicated.
  • Supervise and delegate to the Physical Therapy Assistant and rehab staff.
  • Collaborate with interdisciplinary teams and residents' families on care goals.
  • Plan and conduct therapy programs per physician prescriptions.
  • Assess equipment needs and recommend modifications when appropriate.
  • Record resident progress and adjust interventions as needed.
  • Discharge from services once goals are achieved.
  • Design home exercise or restorative programs for post-discharge care.
