Job summary

Concentra is seeking a Physical Therapist to provide direct patient care, perform evaluations and develop treatment plans in a per diem capacity at our Bridgeview center. The role emphasizes multidisciplinary collaboration, accurate documentation, and patient education to support recovery and functional goals.

The ideal candidate will hold a PT degree with preferred orthopedic outpatient experience and a track record of professional development and compliance with HIPAA/PHI standards.

Qualifications

• Bachelor's Degree from an accredited Physical Therapy program.
• Masters' Degree from an accredited Physical Therapy program, preferred.
• Ortho-outpatient experience, preferred.

Responsibilities

• Deliver high quality patient care and perform evaluations of muscular, skeletal, neurological and other physical problems.
• Coordinate treatment with multidisciplinary teams and ensure proper documentation.
• Plan, implement and adjust physical therapy treatment programs for recovery and maximum function.
• Educate patients on proper care, exercise programs and assistive devices.
• Collaborate with physicians to ensure comprehensive service delivery.
