Job summary

Renewal Rehab, part of the Enhance Therapies family, is hiring a Physical Therapist - Per Diem to join our SNF/Long-Term Care team in Savoy, IL. The role offers flexible scheduling and per diem compensation. A licensed PT is required to apply.

You will evaluate patients, develop personalized care plans, and collaborate with clinicians to achieve quality outcomes. Documentation and adherence to physician orders are essential for regulatory compliance.

Qualifications

• Must be a licensed Physical Therapist in the state of practice.
• Degree from an APTA-accredited Physical Therapy program (BS, MS, or DPT).
• Strong communication and organizational skills.

Responsibilities

• Evaluate and treat patients with mobility, strength, balance, and functional impairments.
• Create and implement personalized care plans.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet all regulatory requirements.
