Job summary

Huntington Hospital in Pasadena, CA is seeking a Per Diem Physical Therapist to evaluate and treat patients in the acute care setting. The role may require occasional floating to the Inpatient Acute Rehabilitation Unit and collaboration with pediatric teams for CCS referrals.

Qualified candidates will have CA licensure as a Physical Therapist, a current BLS, and a graduate degree from an accredited PT program.

Qualifications

• Graduate of an accredited Physical Therapy Program.
• Current Physical Therapy License issued by Physical Therapy Board of California.
• Current Basic Life Support Provider (BLS) certification issued by American Heart Association.

Responsibilities

• Coordinates care to meet identified Physical Therapy needs and participates in management of patient care across the health care continuum.
• Preceptor for related staff and students; collaborates with pediatric department staff and CCS for referrals and continuity of care.

Expectations:

The primary setting for this position is the acute care hospital setting. May require occasional floating to the Inpatient Acute Rehabilitation Unit.

Evaluates, plans and administers treatments for disabilities, injuries and diseases in accordance with a physician’s order. Responsible for coordination of care to meet identified Physical Therapy needs of the patient/family; participates in the management of patient care to include identification, monitoring care delivery, advocating for the patient, evaluating outcomes, facilitating patient care across the health care continuum, and assists other team members with meeting patient needs. Is preceptor for related staff and students. Requires California State Physical Therapist License.
This position may require flexibility of hours.

EDUCATION:
Graduate of an accredited Physical Therapy Program.

EXPERIENCE/TRAINING:
Determined by patient care area with 1 year related practice experience preferred.

LICENSES/CERTIFICATIONS:
Required:
Current Physical Therapy License issued by Physical Therapy Board of California
Current Basic Life Support Provider (BLS) issued by American Heart Association

course of treatment and in discharge planning. A Physical therapy representative will participate in team meetings regarding pediatric patients who are receiving Physical therapy intervention.

1. In collaboration with the pediatric department clinical nurse specialist and/or nurse educator, the physical therapist will provide education and consultation for pediatric staff development regarding PT related topics pertinent to overall patient care.




2. Physical Therapy collaborates with other providers to facilitate the continuum of care.
   Example: The pediatric PT will coordinate with a CCS case manager and/or a representative of the CCS Medical Therapy Program for referral to a CCS-paneled provider or CCS MTU for the patient with a CCS-eligible condition who may continue to require physical therapy services after hospital discharge. The pediatric physical therapist will communicate with other health, education and social service providers such as the Local Education Agency, Regional Center or pediatric inpatient/outpatient programs to assure continuity of intervention.




3. Physical Therapy is performed by Physical therapists (PT) and Physical therapist assistants (PTA) that are licensed by the state of California. There is at least one Physical therapist that is CCS-paneled on the hospital staff.

Pay Range: The estimated base rate for this position is $59.47 - $79.95.

Additional individual compensation may be available for this role through differentials, extra shift incentives, bonuses, etc. Base pay is only a portion of the total rewards package, and a comprehensive benefits program is available for qualifying positions.
