Job summary

Lutheran Senior LIFE is hiring a Per Diem Physical Therapist for the PACE program in Jersey City. The role requires assessing, planning, and administering rehabilitative programs to improve mobility and reduce pain. You will work flexible hours 8:30a-5p, Mon-Fri, with daily patient interaction and care coordination.

The ideal candidate has a DPT degree and an active PT license, plus 2+ years of experience. Vaccination against Covid-19 is required, and EOE.

Qualifications

• Graduate of a physical therapy curriculum approved by applicable associations.
• Active state Physical Therapist license.
• 2+ years hands-on experience preferred.

Responsibilities

• Assess and plan rehabilitative programs to improve mobility and reduce pain.
• Administer therapy sessions and adapt plans to participant needs.
• Collaborate with the PACE program team to coordinate care.
• Document progress and communicate with participants and care teams.

We are hiring a Per Diem Physical Therapist for our PACE program! This position would fill in as needed between the hours 8:30a-5p Mon-Fri.

The Physical Therapist Assesses, plans, organizes, and administers rehabilitative programs that improve mobility, relieve pain, increase strength, and improve or correct disabling conditions resulting from disease or injury.

Basic Qualifications

Education/Training/Certifications: Graduate of a physical therapy curriculum approved by one of the following: The American Physical Therapy Association; The Committee on Allied Health Education and Accreditation of the American Medical Association; The Council on Medical Education of the American Medical Association and the American Physical Therapy Association.

Current Physical Therapist state license

Skill(s): Excellent verbal and written communication, service orientation, critical thinking, instructing, listening & time management.

Experience: Minimum 2 years practicing as Physical Therapist; or equivalent combination of education and experience.
