Job summary

Hayward Hills Health Care Center is seeking a Physical Therapist to evaluate and treat residents with temporary or long-term physical disabilities. You will apply exercise, heat, ultrasound, massage and other therapies to relieve pain and restore function, under physician orders and within scope of practice.

The role includes planning care, performing tests, instructing patients and families, maintaining records, and possibly supervising assistants per state guidelines.

Qualifications

• Bachelor or Master in Physical Therapy required or preferred.
• 1 year of clinical experience preferred.
• Current PT license issued by state board.

Responsibilities

• Evaluate and treat residents with neuromuscular and musculoskeletal issues.
• Administer and interpret tests for strength, ROM, and function.
• Provide direct patient treatment per plan of care.
• Instruct patients and caregivers in use of mobility aids.
• Maintain patient records and documentation.
• Supervise PTA and rehab staff as allowed by state guidelines.

Job description

SUMMARY

Physical Therapist (PT) evaluates and treats residents and patients with temporary or long term physical disabilities to relieve pain, restore function, and promote healing by applying the therapeutic properties of exercise, heat, ultrasound, massage and other forms of treatment.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

Under a physician’s order, performs the following within scope of practice and federal guidelines:

• Evaluates, plans and administers treatment to patients with problems related to neuromuscular and musculoskeletal systems.
• Administers and interprets tests and measurements for muscle strength, coordination, range of motion, and respiratory and circulatory efficiency.
• Provides direct resident treatment in accordance with an established plan of care.
• Instructs patients, families, and caregivers in the care and use of wheelchairs, braces, canes, crutches and other devices.
• Maintains patient records.
• Devises special tools and treatment for the specific needs of patients.
• Conducts home assessments as appropriate to ensure discharge of residents to a safe environment.
• Completes required forms and documents in accordance with company policy and state and/or federal regulations.

ENTRY QUALIFICATIONS

• Bachelor of Science or Master of Science degree in Physical Therapy.
• One (1) year of clinical experience preferred.
• Current license to practice Physical Therapy issued by State’s Board of Allied Health Professionals.

SUPERVISORY RESPONSIBILITIES

May provide onsite supervision to Physical Therapist Assistants, Rehab Aides, and provisional physical therapists per state practice guidelines.
