Job summary

Renewal Rehab is seeking a licensed Physical Therapist for a per diem position in a SNF/Long-Term Care setting. The role focuses on evaluating and treating mobility, strength, balance, and functional impairments, with individualized care plans and collaborative team effort.

The organization offers mentorship, flexible scheduling, and ongoing professional development as part of Enhance Therapies. New grads are welcome and immigration support is available for eligible candidates.

Qualifications

• Must be licensed Physical Therapist in the state of practice.
• New grads welcome! Mentorship available.
• Degree from an APTA-accredited Physical Therapy program (BS, MS, or DPT).
• Current license as Physical Therapist in the state of practice.
• Strong communication and organizational skills.

Responsibilities

• Evaluate and treat patients with mobility, strength, balance, and functional impairments.
• Create and implement personalized care plans.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet all regulatory requirements.
• Ensure all required physician orders are obtained and up to date.
